Paragon Rehabilitation, a subsidiary of Trilogy Health Services LLC, is currently seeking a licensed Therapy Manager/ Rehab Manager/ Therapy Director at Wellbrooke of Crawfordsville , a dynamic and innovative Skilled Nursing and Assisted Living health campus in Crawfordsville, IN .
The Therapy Program Director will lead the therapy/rehab team in our health campus. Candidates must hold an active or pending therapy state license in order to apply for this position.

Graduate of an accredited Bachelor's or Master's degree program
Hold a current license as a physical or occupational therapist or speech-language pathologist, or be a graduate of a two year associate degree program and hold a current license as a physical or occupational therapy assistant
Proven track record of successful management experience and successful patient care experience is required
Experience working with a geriatric disabled population preferred
Provision of own transportation is necessaryEstimated Salary: $20 to $28 per hour based on qualifications.
